EXPRESS CONSENT FOR THE USE AND PROCESSING OF PERSONAL DATA

V & C Odontólogos SAC (hereinafter, THE COMPANY) within the framework of Law No. 29733, the Personal Data Protection Law (hereinafter, the Law), its regulations, amendments, and supplementary rules in force, has informed me that it will be RESPONSIBLE for the use, processing, and security of the personal data that I voluntarily provide to it or that it collects physically, orally, or electronically, through publicly accessible sources or from third parties and/or database query entities domiciled in Peru or abroad, whether natural or legal persons, private or public. Said data will be stored in the Data Bank of V & C Odontólogos, of which it is the owner.

OF THE PERSONAL DATA

THE COMPANY will process my personal data according to the following classification:

  1. Identifying data: first and last names, DNI, RUC, passport, home address, landline and mobile phone, email address, image, voice, signature.
  2. Personal characteristics: marital status, date of birth, nationality, sex, profession, age, academic data, data of beneficiaries, contact person data.
  3. Sensitive data: information relating to physical or mental health, emotional or family life, income, union membership, fingerprint.
 

OF THE PURPOSE

THE COMPANY has informed me that the purpose for which it collects my personal data is:

  • To execute, manage, and administer the contractual relationship;
  • Commercial and informational purposes, offering products and/or services, whether those of THE COMPANY and/or of the companies that are part of the same economic group and/or its national and international, public or private business partners, sending advertising, offers, promotions, benefits, commercial campaigns, for which they may use, among other means, call centers (call centers), telephone calling systems, sending text messages to mobile phones or mass and/or personalized emails, sending magazines and/or catalogs and/or any other similar means of communication existing or to be created.
  • To use, provide, and/or transfer this information to the service providers contracted by THE COMPANY, as well as to authorities and third parties authorized by law.
  • To fulfill statistical and historical purposes for THE COMPANY, and/or the companies that are part of the same national and international economic group;
  • To assess my creditworthiness and payment capacity;
  • To evaluate any request that I make and/or may make;
  • To conduct surveys about the services of THE COMPANY /or the companies that are part of the same economic group.

THE COMPANY may transfer my personal data within the national territory, by any means, to its subsidiaries, business partners, related companies, its parent or controlling company, its service providers, whether natural or legal persons, Peruvian or foreign.

OF THE PERIOD OF PROCESSING OF PERSONAL DATA

My personal data will remain stored while their use and processing are necessary to fulfill the purposes described above, or for up to five (05) years after the contractual relationship has ended.
