Wisdom teeth

When should wisdom teeth be removed?

Wisdom teeth usually erupt in late adolescence or young adulthood. Some find enough space, while others remain impacted, tilted, or partially covered by the gum.

The decision to remove them depends on their position, the available space, the presence of symptoms, and their relationship with nearby teeth or anatomical structures.

Pain or swelling

Discomfort in the back of the mouth, swollen gums, or difficulty opening the mouth.

Infecciones repetidas

The gum that partially covers the tooth can trap food and bacteria.

Tilted or impacted position

It may come into contact with the second molar or make cleaning between both teeth difficult.

Cavities that are hard to treat

Its location can make cleaning and access for a proper restoration difficult.

Damage to the neighboring tooth

A poorly positioned third molar can promote cavities or resorption in the nearby tooth.

Orthodontic indication

It may be recommended when the extraction is part of an individualized orthodontic plan.

A wisdom tooth without symptoms does not always need to be removed.

First we evaluate its position, hygiene, prognosis, and relationship with nearby structures.

Evaluation and diagnosis

When might you need oral surgery?

Oral surgery includes procedures ranging from a simple extraction to the treatment of impacted teeth, wisdom teeth, root remnants, and other cases that require more careful planning.

Dientes fracturados

An extensive fracture can make it impossible to restore the tooth, especially when it involves the root or lies below the gum line.

  • Fracturas profundas.
  • Teeth with a poor prognosis.
  • Pain when biting or chewing.

Infecciones recurrentes

A tooth with a persistent infection can cause pain, swelling, a bad taste, or repeated episodes that require treating the cause.

  • Swelling of the gum or face.
  • Discharge or bad taste.
  • Episodes of recurring pain.

Restos radiculares

Some previous extractions can leave root fragments that need to be removed before a prosthesis or an implant, or because of the risk of infection.

  • Retained roots.
  • Localized pain or swelling.
  • Preparation for implants.

Dientes retenidos

Canines or other teeth may remain inside the bone and require surgical exposure or extraction, depending on the orthodontic plan and the position of the tooth.

  • Canines that did not erupt.
  • Dientes en posición anómala.
  • Treatments coordinated with orthodontics.

Preparation for other treatments

Some teeth must be removed before starting orthodontics, oral rehabilitation, prosthesis placement, or dental implant treatment.

  • Orthodontic planning.
  • Preparation for prostheses.
  • Comprehensive rehabilitation.

Not all extractions need surgery

The type of procedure depends on the position of the tooth, its relationship with the bone, the roots, and nearby anatomical structures. That is why we first carry out a clinical evaluation and request imaging when indicated.

01
Technology at the clinic

We perform 2D and 3D studies according to each patient's clinical needs, without having to refer them to another center.

01

Clinical evaluation

First we review your history, symptoms, and oral condition. This evaluation helps identify the area to be treated and determine which imaging study can provide useful information.

02

Panoramic X-ray

It offers a general view of the jaws, teeth, and bone. In many cases it provides the initial information needed to assess an extraction.

Initial study depending on the case
04

Personalized surgical plan

With the clinical and radiographic information, we define the type of procedure, the necessary care, and the appropriate alternatives for each case.

Do all patients need a 3D tomography?

No. In some cases a panoramic X-ray provides the necessary information. A tomography is requested when it can provide additional data relevant to planning.

Anxiety management

Local anesthesia and conscious sedation: they are not the same

Feeling nervous before an extraction or oral surgery is common. We talk with you, answer your questions, and evaluate what measures can help you go through the procedure with greater peace of mind.

01

Anestesia local

It is administered in the area to be treated to temporarily block sensation. The patient stays awake and may feel pressure, movement, or vibration during the procedure.

Sedation is not automatically indicated.

The decision depends on your general health, medication, level of anxiety, type of surgery, and professional evaluation.

Prior evaluation

When may it be considered?

Ansiedad dental intensa

When fear makes it difficult to stay calm during treatment.

Reflejo nauseoso marcado

When the gag reflex makes dental care difficult.

Procedimientos seleccionados

In certain surgeries or longer treatments.

Instructions that may be requested
Complete medical history
Fasting, if indicated
Companion, when applicable

Clinical evaluation is essential

Report any illnesses, pregnancy, allergies, regular medication, anticoagulants, and history of reactions to anesthetics or sedatives.

Recovery and follow-up

Care after oral surgery

Protecting the clot, following the prescribed medication, and respecting the surgeon's instructions support proper recovery. Care may vary depending on the procedure and each patient.

2–3 días
Initial recovery

Manage the discomfort

Swelling may increase during the first 48 to 72 hours. Use external cold only when indicated and follow the medication, diet, and hygiene guidelines you received.

Follow-up
Follow-up

Check the healing

Come on the indicated date to assess progress, resolve questions, and have the sutures removed when the material used requires it.

Recomendaciones

What you should do

Soft diet and hydration

Prefer soft, cool, or lukewarm foods and drink water directly from the glass.

Higiene cuidadosa

Gently clean your other teeth and follow the specific instructions for the operated area.

Medication as prescribed

Follow the doses and schedules. Do not stop, combine, or substitute medications on your own.

Rest and follow-up

Reduce activity during the initial recovery and attend the scheduled follow-up.

Precauciones

What you should avoid

Spitting, sucking, or rinsing forcefully

During the first 24 hours these actions can dislodge the clot and restart bleeding.

Smoking or drinking alcohol

They can interfere with healing and increase the risk of complications.

Hot, hard, or irritating foods

Avoid crunchy, very hot, spicy foods, foods with seeds, or foods that are hard to chew.

Intense exercise or touching the wound

Avoid exertion and do not touch the area with your fingers, tongue, toothbrush, or other objects.

When to ask for help

Signs that require attention

Sangrado persistente

Heavy bleeding that does not decrease after applying firm pressure with clean gauze.

Pain or swelling that gets worse

Intense pain, progressive swelling, fever, discharge, bad odor, or persistent bad taste.

Possible reaction to medications

A rash, swelling, or other reaction after taking a medication requires assessment.

Difficulty breathing or swallowing

Seek emergency care immediately. Do not wait for a reply on WhatsApp.

The instructions given by your surgeon take precedence over these general recommendations, because each patient and procedure is different.

Orthodontics and surgery

Exposure of canines or impacted teeth

Some teeth remain inside the bone and may require surgical exposure to facilitate their traction, in coordination with orthodontic treatment.

03
Tejidos blandos

Labial or lingual frenectomy

It is evaluated when a frenulum limits mobility or interferes with the tissues, hygiene, or orthodontic treatment. The indication is determined after a functional assessment.

04
Diagnosis of lesions

Oral tissue biopsies

When a lesion needs to be studied, a tissue sample may be taken and sent to a specialized laboratory for pathological analysis.

05
Oral pathology

Cysts and selected lesions

Soft tissue or bone lesions require clinical evaluation and imaging. Their management or referral depends on the diagnosis, size, location, and complexity.

06
Implantology

Surgical preparation for implants

Before placing an implant, a planned extraction, conditioning of the area, or another complementary procedure may be indicated, depending on the volume and condition of the tissues.

Personalized evaluation

First we confirm the diagnosis and the alternatives

Not all impacted teeth, lesions, or root remnants require the same treatment. We review your symptoms, the clinical examination, and the images before recommending a procedure.

We answer your questions

Frequently asked questions about oral surgery

These answers provide general guidance. The diagnosis, duration, recovery, and cost depend on the clinical evaluation and the characteristics of each procedure.

01 Is wisdom tooth surgery painful?

Before starting, local anesthesia is administered to block sensation in the area. During the procedure you may feel pressure, movement, or vibration. If you feel pain, you must tell the professional immediately. Afterward, discomfort and swelling may appear and are managed by following the instructions you receive.

02 How long does a wisdom tooth extraction take?

The duration depends on the position of the tooth, the shape of its roots, the amount of bone covering it, and its proximity to important anatomical structures. A simple extraction may take less time than a completely impacted tooth. The approximate time is estimated after reviewing the images.

03 How much does wisdom tooth surgery cost in Lima?

The cost varies depending on the position of the tooth, the surgical complexity, the imaging needed, and complementary procedures. To provide an accurate quote, it is necessary to carry out an evaluation and review a panoramic X-ray or tomography when indicated.

04 How many days do recovery and swelling last?

Recovery time depends on the complexity, the number of teeth treated, and the patient's usual activity. Swelling may increase during the first 48 to 72 hours and then begin to decrease. Some people resume light activities the next day, while other cases require several days of rest.

05 Can all four wisdom teeth be extracted at once?

In some patients it is possible to perform all four extractions in a single session. The decision depends on the position of the teeth, the estimated duration, health status, level of anxiety, and expected recovery. In other cases it may be more convenient to divide the treatment into two appointments.

06 Do I need a 3D tomography to have a wisdom tooth extracted?

Not all patients need a tomography. In many cases, the clinical evaluation and a panoramic X-ray provide the necessary initial information. A 3D tomography may be requested to study the roots, the position within the bone, or the relationship with the inferior alveolar canal in greater detail.

07 Can I request sedation for oral surgery?

Conscious sedation may be evaluated in patients with intense anxiety, a marked gag reflex, or selected procedures. It does not replace local anesthesia, and its indication depends on the medical history, regular medication, and type of surgery. Fasting and the presence of a companion are requested only when applicable.

08 When should I contact the clinic or seek immediate care?

Contact the clinic if you have persistent bleeding, pain or swelling that gets worse, fever, discharge, bad odor, or a persistent bad taste. If you have difficulty breathing or swallowing, or signs of a severe allergic reaction, seek emergency care immediately and do not wait for a reply on WhatsApp.
